Most therapists set their rate by guessing, or by copying the person down the street — and then wonder why the numbers feel tight. A sustainable rate works backward from the life you want: your income goal, plus expenses and taxes, divided by the number of sessions you can realistically hold. Why rate-setting is really practice-sustainability
Your rate isn't just a price — it's what determines whether your practice supports your life or slowly burns you out. Undercharging is one of the most common reasons good clinicians leave private practice. Setting a rate on purpose is an act of sustainability, for you and for the clients who need you to still be here in five years.
The formula, in plain terms
• Start with your target take-home income. What you actually need to live on.
• Add business expenses. Rent, EHR, licensing, insurance, marketing, supervision.
• Add taxes. Self-employment taxes are significant; set aside a realistic percentage.
• Divide by realistic billable sessions. Not 40 a week — account for admin, no-shows, time off, and sustainable clinical load.
The result is the revenue each session needs to generate. Working backward like this almost always reveals that the sustainable rate is higher than people expect.
Don't forget the hidden factors
A full calendar on paper isn't a full calendar in reality. Build in a buffer for cancellations and no-shows, unpaid admin time, vacation and sick days, and slower seasons. A rate that only works if every slot is filled every week isn't sustainable — it's a setup for burnout.

• Assuming 100% of scheduled sessions will happen and pay.
